On call for after hours inbound calls up to 8pm EST
- Care coordination : coordinate the next level of care services for patients with Home Health, Dialysis, and other Outpatient appointments.
- Act as a point of contact/Liaison between patients, MDs, PAs, NPs and Insurance providers, providers to ensure smooth transition of care.
- Coordinate Outpatient services for patients; Specialist appts, Dialysis, Home Health
- Reinforce patient education in understanding disease process and medication management
